The type of personal information we collect
We currently collect and process the following information:
· Personal identifiers such as name, address and contact number(s).
· Date of Birth.
· Email address.
· Name of Parent or Carer if patient is underage.
· Occupation.
· Insurance provider details.
· Documentation in relation to your injury/condition such as reports from GP’s or Consultant.
How we get the personal information and why we have it
Most of the personal information we process is provided to us directly by you or your health insurance provider. If you are seeing us through your insurance provider then we will share information between each other in relation to your care. We also require your personal information for the following purposes:
· To identify an individual.
· To match individuals to insurance provider policy numbers and authorisation codes.
· To write reports or to update insurance providers regarding your treatment.
· To send information to yourself or to people involved in your care such as a GP or Consultant.
How long do you hold personal information and how do you dispose of it
We store your personal information for eight (8) years if an adult or until the 25th birthday if a child, but this can increase if there are specific circumstances.
We will dispose any paper personal information by cross shredding or incineration once your information is no longer needed or expired. For personal information held on electronic devices, we follow all methods of destruction which meet the ICO requirements. The methods include wiping, degaussing or secure destruction of hardware (shredding).
In the instance of any legal claims that arise from you using our services, then your data will be stored for as long as is required or advised by our legal counsel.
How we protect your personal information
Primal Physio has taken steps according to Data Protection Laws to protect your personal information by using the following methods:
· Locking all computer screens when away from them
· Ensuring all computers are regularly updated with the latest antivirus software.
· Using two factor authentications for key systems such as email.
· Changing passwords regularly on all our systems.
· Our third party providers of systems used to process your personal data are compliant with data protection laws and requirements, and also have effective data restore capabilities to ensure your data can be recovered.
· Providing data protection training to all staff.
Your data protection rights
Under data protection law, you have rights including:
· Your right of access - You have the right to ask us for copies of your personal information.
· Your right to rectification - You have the right to ask us to rectify personal information you think is inaccurate. You also have the right to ask us to complete information you think is incomplete.
· Your right to restriction of processing - You have the right to ask us to restrict the processing of your personal information in certain circumstances.
· Your right to object to processing - You have the the right to object to the processing of your personal information in certain circumstances.
· Your right to data portability - You have the right to ask that we transfer the personal information you gave us to another organisation, or to you, in certain circumstances.
You are not required to pay any charge for exercising your rights. If you make a request, we have one month to respond to you.
